The SINGLE ANASTOMOSIS DUODENAL-ILEAL BYPASS WITH SLEEVE (SADI-S)is a bariatric surgical procedure that combines two techniques: sleeve gastrectomy and duodenal-ileal bypass. It is a modified form of the traditional duodenal switch procedure.
During the SADI-S procedure, a large portion of the stomach is removed, leaving behind a smaller sleeve-shaped stomach. This sleeve restricts the amount of food that can be consumed, leading to a reduced calorie intake.
In addition to the sleeve gastrectomy, a bypass is created in the small intestine. The first part of the small intestine, called the duodenum, is divided, and the lower part of the intestine is then connected to the ileum, bypassing a significant length of the small intestine. This bypass reduces the absorption of calories and nutrients from food.
The SADI-S procedure aims to achieve both restriction (reducing the amount of food intake) and malabsorption (decreasing nutrient absorption) to promote weight loss. By combining these mechanisms, the procedure helps individuals achieve significant and sustained weight loss.
While specific success rates may differ across studies, research suggests that the SADI-S procedure can lead to significant weight loss. Reported excess weight loss (EWL) percentages range from approximately 60% to 80% at one year after surgery. Some studies have also demonstrated sustained weight loss and positive outcomes over longer follow-up periods.
Additionally, the SADI-S surgery has been associated with improvements or resolution of obesity-related comorbidities, such as type 2 diabetes, hypertension, and dyslipidemia.
